Enrollment Information
The following information is needed for enrollment:
- Birth Certificate
- Social Security Card
- Shot Records ( Please see below)
- Withdrawal documents from your last school
- Unofficial transcript or last report card
- Proof of residence (i.e., household utility bill, mortgage or lease documents)
- ID of person enrolling student
- Notarized guardianship papers (if student is not living with parents)
***Texas Legislative Law on School Vaccines***
Military & other Texas schools are allowed 30 days to get vaccine records. On the 31st day the student can not be at school without proof of vaccines.
Out of state & non military CAN NOT start school without proof of vaccines. You must have current and up to date shot records.
